Ask any care coordinator who has worked with a poorly configured RPM program what their biggest complaint is, and you'll hear some version of the same answer: too many alerts, most of them meaningless. The experience is exhausting. More importantly, it's dangerous. Alert fatigue in clinical settings is a documented patient safety risk — when staff become habituated to ignoring alarms, they're more likely to miss the ones that matter.

The alert overload problem in wearable-based RPM is largely self-inflicted. It comes from deploying continuous monitoring technology without redesigning the clinical decision logic around it. More data is not automatically better. More data without a thoughtful alert architecture is a liability.

Why Default Thresholds Don't Work

Most RPM platforms ship with default alert thresholds: flag a blood pressure reading above 140/90, a heart rate above 100, an oxygen saturation below 94%. Those numbers are clinically defensible as population-level cutoffs. They're not defensible as individual patient thresholds.

A 68-year-old with COPD whose baseline SpO2 has been 93-94% for years doesn't need an alert every time their saturation reads 93%. A cardiac patient on rate-limiting medications whose resting heart rate runs 62-68 needs a lower tachycardia threshold than a healthy 45-year-old. Population defaults applied to individual patients generate enormous quantities of false positives — and a care team that receives 40 alerts on a Tuesday morning will, rationally and inevitably, start scanning them rather than reading them.

The solution isn't more sophisticated algorithms — it's more careful baseline calibration at enrollment. Before you set a patient's alert thresholds, you need 10 to 14 days of baseline data collected under normal conditions. Not pre-calculated from charts. Actual transmitted readings from their assigned device, in their home environment, during their typical daily routine.

Tiered Alert Architecture

Not every deviation requires the same response, and designing your alert system to treat all thresholds equally is a fast path to burnout. A tiered structure separates the response pathway from the detection event — so the care coordinator's attention is reserved for the situations that actually need it.

A workable three-tier model: Tier 1 generates a dashboard flag visible to care coordinators during scheduled review periods — no notification, no interrupt, just a marker for the next review. Appropriate for readings that cross threshold once or deviate modestly from baseline. Tier 2 generates a notification to the assigned care coordinator during business hours — requires acknowledgment and response within four hours. Appropriate for sustained threshold crossings (two or more readings in 24 hours) or moderate deviations in high-risk patients. Tier 3 generates an immediate alert with escalation to on-call clinical staff — appropriate for critically abnormal values or sustained deterioration trends in any enrolled patient.

The clinical criteria for each tier need to be defined per patient cohort and reviewed periodically. Tier 3 criteria for a hypertension patient look different from Tier 3 criteria for a post-cardiac surgery patient.

Data Aggregation Before Alerting

Wearables, particularly those measuring heart rate and activity, generate data at high frequency. A smartwatch-style device might transmit heart rate every 30 seconds during exercise. No clinical system should be alerting on individual readings from high-frequency sensors. The data needs aggregation — a 5-minute rolling average for heart rate, an hourly summary for activity levels — before it enters the alert logic.

Blood pressure cuffs and glucometers operate differently: readings are episodic and intentional. A single reading that crosses threshold is more meaningful than a single high-frequency sensor reading. Even so, a policy of alerting on two consecutive threshold crossings rather than one reduces false positive rates substantially without meaningful delay in detecting genuine deterioration.

Context Filtering

Heart rate spikes during physical activity aren't clinically interesting. A system that doesn't filter activity context from heart rate alerts will send care coordinators notifications every time an enrolled patient walks up stairs. Most modern RPM platforms support activity context tagging — if the patient's device is registering high step counts or elevated activity, the heart rate alert logic should account for it.

Similarly, blood pressure readings taken immediately after waking or immediately after physical activity are physiologically different from resting readings. If your devices support activity state tagging, build that into your alert configuration. If they don't, at minimum educate patients to take their readings at consistent times — ideally in the morning after five minutes of rest — and make that protocol explicit in enrollment documentation.

Reviewing Your Alert Performance

Every RPM program should be tracking the ratio of alerts generated to interventions that required clinical action. If your program generates 300 alerts per month and 8 of them result in a medication change or care escalation, your false positive rate is roughly 97%. That is too high. Not because precision matters more than recall — missing a genuine deterioration is worse than receiving a false alarm — but because a 97% false positive rate means your care team has normalized ignoring alerts, and the 3% that matter are buried in noise.

Review alert performance monthly for the first six months of any new program. Adjust thresholds. Add context filters. Remove cohort-level defaults and replace them with patient-specific baselines. Alert architecture is not a one-time configuration — it's an ongoing clinical quality management task.

The goal isn't to minimize alerts. It's to make every alert meaningful enough that when it arrives, the care team acts on it immediately.
